Good paper from @MaryDixonWoods et al on operational failures in primary care. Having spend hours of ‘compensatory labour’ as a carer, would be interesting to see a similar framework used to look at operational failures from the patient/care point of view.

Really recommend reading this paper. Operational failures in primary care have a huge impact on both doctor and patient - this paper offers real insight into the burden on GPs, and how their extra work to correct failures is hidden even as its done.
